Wisdom Tooth Extraction Recovery: A Day-by-Day Guide

Wisdom Tooth Extraction Recovery

Around 80% of people will need their wisdom teeth removed at some point in their lives. The recovery experience varies depending on how complex the extraction was — a simple erupted tooth heals far faster than a deeply impacted one requiring surgical removal. But regardless of complexity, the trajectory is predictable. Here is exactly what to expect, day by day.

Day 1 — Surgery Day

What is normal today:

  • Bleeding is normal for 2-4 hours after extraction — keep biting firmly on the gauze pad provided
  • Numbness from the anaesthetic lasts 2-6 hours — be careful not to bite your cheek or lip
  • Some oozing of blood mixed with saliva — this can make it look like more bleeding than it actually is

What to do:

  • Rest completely — avoid any strenuous activity or bending over
  • Apply cold compress to your cheek — 20 minutes on, 20 minutes off — to minimise swelling
  • Take all prescribed medications as directed, starting immediately
  • Eat only liquids and very soft foods: idli water, fruit juice, cold yogurt, thin dal
  • Elevate your head when lying down — this reduces bleeding and swelling

Days 2-3 — Peak Swelling

This is typically the most uncomfortable phase, but it is normal and expected. Swelling peaks at 48-72 hours and often looks significantly worse than it feels. You may also notice bruising appearing on your cheek or even spreading towards your neck — this is simply blood tracking under the skin and is harmless.

  • Mild to moderate ache is normal — manage with prescribed painkillers taken regularly
  • Switch from cold compress to warm compress on Day 3 to help resolve swelling
  • Soft foods: curd rice, mashed banana, soft dal, mashed potato, room-temperature soups
  • Do NOT rinse your mouth forcefully — the blood clot in the socket is fragile and critical for healing
  • Do NOT use a straw — the suction force can dislodge the clot (causing dry socket)

Days 4-5 — Swelling Begins to Subside

You will notice a meaningful reduction in swelling from Day 4 onwards. The ache should also be reducing at this point. This is when you can begin gentle warm saltwater rinses — half a teaspoon of salt dissolved in a glass of warm water. Tilt your head gently to let the rinse flow around the socket, then let it fall out of your mouth. Do not spit forcefully.

You can slowly reintroduce slightly more textured soft foods — soft rotis torn into small pieces, soft cooked vegetables, eggs. Continue avoiding anything hard, crunchy, or sticky.

Days 6-7 — Healing Well

By the end of the first week, most patients feel 70-80% normal. The socket area will be visibly healing — you may notice pink granulation tissue forming, which is a sign of healthy healing. If sutures were placed, dissolvable sutures typically begin to loosen around this point. Non-dissolvable sutures will be removed at your follow-up appointment at Emitsmile.

Week 2 and Beyond

Most patients return fully to their normal diet within two weeks. The socket continues to close and fill in over the next 3-6 months as bone gradually fills the extraction site — this happens invisibly beneath the gum surface and requires no action from you.

What to Eat — and What to Avoid

Eat These

  • Curd rice (room temperature)
  • Mashed banana, mashed potato
  • Yogurt and smoothies
  • Soups (not too hot)
  • Soft idli, upma
  • Dal (not too spicy)
  • Scrambled eggs

Avoid These

  • Hard/crunchy foods (biscuits, chips, raw carrots)
  • Sticky foods (toffee, caramel, chewing gum)
  • Spicy or very hot foods
  • Alcohol (interferes with healing)
  • Using a straw (suction dislodges clot)
  • Seeds and small grains (get trapped in socket)

Warning Signs — When to Call Us Immediately

Dry Socket — Call Immediately If:

  • Severe, increasing pain beginning on Day 3-4 instead of decreasing — this is the hallmark symptom
  • Persistent bad odour or unpleasant taste from the socket
  • Visible empty-looking socket with no clot formation

Signs of Infection — Call Immediately If:

  • Fever above 38°C (100.4°F)
  • Swelling that is increasing after Day 3 instead of reducing
  • Pus or discharge from the socket
  • Difficulty opening your mouth (trismus) that is worsening
